Title: What are your experiences with the PDF Preview?

I'm considering MC for working on large sets of often large (>1000-page) annotated PDFs in preview and wonder if any similar user has found MC successful for this? I've tried alternatives but discovered issues of reliability and performance on some large files e.g. a new preview blocking input.

Also does MC use the Windows iPreviewHandler, or what?

PDF preview in MC is using the Windows Shell PreviewHandler (IPreviewHandler interface).
So the preview is dependend on what PDF preview handler that is installed.. It is only tested and verified with the build in viewer in Windows

Consider QuickLook (https://github.com/QL-Win/QuickLook). It supports MC. But I did not estimate its performance with large files.
